This notice describes how information submitted through this website is handled. It does not replace the Notice of Privacy Practices provided by the office for patient health information.

Information we collect

The appointment request form asks for your name, contact details, preferred contact method, and general area of interest. Online delivery is not currently configured: the form does not send or save your request, and displays a notice directing you to call or email the office. Please do not enter medical details.

How it is used

Please call or email the office for appointment inquiries. Before online delivery is enabled, this notice must be updated to describe the actual recipient, services used, storage, and retention. Please contact the office for the practice's Notice of Privacy Practices, which describes how patient health information is handled.

Third-party services

This website loads fonts from Google Fonts, provider photographs from Wix, and embedded maps from Google Maps. Those services receive connection information, such as your IP address, when their content loads. Appointment form values are not included in those requests. Their own privacy policies apply.

Email and phone

Standard email is not a secure channel. Please avoid sending sensitive health information by email.
